Community Housing

This program is focused on adults, ages 18 and older with a diagnosis of chronic mental illness as verified by a psychiatrist, and have had several hospitalizations. Tenants must also require coordinated services because of substantial impairment in functioning. This housing program is partially funded by HUD. It is a supported program, which means that we are not funded for 24 hour services. People that reside here must be able to live independently and not require skilled nursing services. Services provided to tenants are living skills training, counseling, case management, psychiatric services, medication monitoring, transportation, representative payee services, and community support. We are able to service 26 people in this program, between the 2 sites, Bridgeview and Parkview.
